Paperwork

Returning Athletes:

Completed paperwork packets must be received on or before the date of your Tryout/Swim Test (10/7 or 10/10).

New Athletes:

You must provide a current physical and the Transy Consent Form (divers only) on or before your Try Out date (10/7) in order to be allowed in the water. The final roster will be announced on or before 10/18 and fully completed packets must be received by the end of the first week of practice (10/21).

Fall Sport Athletes:

If you participated in a fall sport for Henry Clay, please indicate which sport so we can verify your paperwork is on file. You are still required to complete the online registration, the swim & dive athlete contract, and the Transy consent form (divers only).
